Purchase Request Form

This form is used to suggest that the Clearwater Public Library System purchase an item which it does not own. Please follow the steps below to successfully submit your suggestion. Thank you!

1. Check the Online Library Catalog to verify that Clearwater Public Library does not own the desired item. If the item is newly published (and especially if it is a bestseller), it may already be on order. We do not always acquire titles immediately upon their publication.
2. If the item is not listed in either source, please fill out the following form and our Collection Development librarian will review your suggestion.
3. If the library chooses not to purchase the item, our Interlibrary Loan Department will attempt to obtain it from another library through an Interlibrary Loan Request. We do not guarantee that the title is available or that other libraries will be willing to fulfill our request.

PLEASE NOTE:

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released to a public-records request, do not send electronic mail to this entity or use this form. Instead, please stop into any one of our locations or contact us by phone or in writing.

We require a minimum of an author and title to consider your suggestion. For videocassettes, please enter the director's name, if known, or enter "NA". For magazines, please enter the publisher's name, if known, or enter "NA". Thank you!
