Below are the primary forms and applications used by the City of Clearwater Parks and Recreation Department. To submit a form, please follow these steps:

1. Read all instructions carefully
2. All forms must be printed on white 8-1/2" x 11" paper
3. Complete the entire form in blue or black ink
4. Have the form notarized, if necessary
5. Submit the form and any other required paperwork to the Parks & Recreation Department
