This form is used to request the attendance of the mayor, city council, or city manager at an event. Submitting this form allows the city to review the request, understand the purpose of the event, and coordinate scheduling and logistics. Requests are reviewed based on availability. Submission of this form does not guarantee attendance.

A calendar hold may be requested in advance of submitting this form; however, holds must be emailed to the city executive assistants.
